IRS Employer Search Instructions

Instructions for completing the required 4506-T form for the IRS Employer Search

The candidate will need to enter the following information:

  • Box 1A: Complete Name as it appeared on the tax return for the tax year(s), i.e., 20XX being requested. If the individual received a 1099 related to work performed under a company name that they established with a Federal ID Number (FEIN), then they would include the Company name on that line.

  • Box 1B: Complete SSN, if filed as an individual, or EIN (Employer Identification Number), if filed under a company.

  • Box 2A: Leave blank.

  • Box 2B: Leave blank.

  • Box 3: Candidate’s current name and address if different from box 1A.

  • Box 4: Candidate’s previous address shown on last return filed if different from box 3.

  • Box 5: Do not change or modify this section.

  • Box 6: Enter either W-2 or 1099. Only one of these form types is allowed per 4506-T. The candidate needs to enter which type of tax documents they received for the year(s) being requested. If candidate received both types of forms in one year, multiple 4506-T forms will need to be completed.

  • Boxes A, B, C & 7 should remain UNCHECHEKED.

  • Box 8: Should be checked off.

  • Box 9: Include the year(s) from the tax documents specified in box 6 that are being requested. (Up to four years may be requested per 4506-T Form). Example: 12/31/XXXX.

  • Any Crossed out information must be re-written and initialed by the SSN holder.

  • Form must be hand‑signed (in black ink, pen to paper) and dated by the candidate within the past 90 days.

  • If line 1a is a corporation, partnership, estate or trust, the title of the individual signing the form must be included on the line below the signature labeled “Title”.  Otherwise, it can remain blank.

  • Candidate’s phone number is required in the field at the bottom of the form.

  • Spouse’s Signature and date section are only required if it was a joint tax return.
